Enrollment Process with Canada.ca/dental:

- Enrollment Application: Complete the enrollment application available on Canada.ca. Ensure that you provide all necessary information accurately to expedite the enrollment process.
- Documentation Submission: Gather any required documents as specified by Canada.ca and submit them along with your enrollment application. This may include proof of income or disability status.
- Approval Confirmation: Once your enrollment application is processed and approved by Canada.ca, you will receive confirmation of your eligibility for the Canadian Dental Care Plan.
Appointment with Our Practice:
- Schedule an Examination: Contact our office to schedule an appointment for a comprehensive dental examination. During this visit, we will assess your oral health needs and discuss your treatment options.
- What to bring to your appointment: Please fill out the on-line patient history form and bring your Sunlife Welcome Package; including welcome letter and enrollment card.
- Diagnostic Procedures: If necessary, we may perform X-rays and photos to gather additional information for the submission of predeterminations of treatment. These diagnostic procedures help us create a personalized treatment plan tailored to your specific needs.
- Treatment Predetermination Submission: Prior to starting any treatment, we will work with you to submit predeterminations of treatment to Sun life Insurance, ensuring clarity on covered procedures and associated costs.
- Treatment Planning: Once predeterminations are approved, we will work with you to develop a treatment plan that aligns with your oral health goals and fits within the parameters of the Canadian Dental Care Plan.
